By opting into our SMS program, you agree to receive SMS messages from Forward Medical, LLC in accordance with these Terms and The Campaign Registry (TCR) compliance standards.

- By opting in, you provide express consent to receive SMS messages from regarding: appointment reminders, promotions, account updates, and general office updates.
- Opting into the SMS program is voluntary and separate from any other service we provide.
- To opt-in, sign up in person at our client’s facility.
Message Frequency and Types
- Message Frequency: Varies based on account activity and updates.
- Message Types: May include appointment confirmations, reminders, notifications, account updates, and promotions.
- Message and data rates may apply according to your mobile carrier’s standard rates.
Opt-Out Instructions
- You may opt out at any time by replying “STOP” to any message.
- After sending “STOP,” you will receive a confirmation message. No further messages will be sent unless you re-subscribe.

Privacy and Data Security
